Fall/Spring 2011-2012 Registration
$75 – Standard Registration – includes jersey, shorts and socks
$59 - "Early Bird" discount registration during the month of May

Registration is a two step process!

STEP ONE: On-line Pre-registration
All players must pre-register on-line by going to www.eayso.org to complete the internet registration process.


If it is your first time to use the eAYSO system, you will need to create a personal log-in ID using your email address and a unique password. Then select:

"Application for a new player"

If your child played last year, log into the system using the email address and password you created last year. After logging in, update your personal information (address, phone numbers, email address). Then select:

"Application for a returning Player"

It is important that you review and update your child’s personal information, including school name.

Once all your information has been entered, continue with the legal consent form. Select: “I Agree”.

Payment Options:

  1. Online credit or debit card: If you want to use a credit card, click on “Online payments” and follow the instructions to make an on-line payment with your Visa or MasterCard.
  2. You can also pay by personal check. Bring it with you to Mt Ogden Park Bowery on May 14th.
After completing the registration process, select: .

“Print player form”

Select your child’s name. You will see your child’s registration form on the screen, click on the printer icon under the words “Print Form” and request one copy. (Each registration form has two pages).

STEP TWO: Walk-Up Registration Day – May 14th
After you have completed the online registration, please come to Mt. Ogden Park Bowery on Saturday, May 14th from 9:00 AM to 1:00 PM to complete the registration process. Please bring your completed signed forms and proof of payment (or a personal check).

While we will have limited access to computers at the park, in order to save time, you will want to
print your forms at home
AND bring them to the park with you.

Or if you cannot attend the Walk-up Registration on May 14th, you can mail your signed forms along with one of the following:

  1. a check made out to “AYSO”
  2. a copy of your online payment receipt

Refunds
Refunds may be given to children who have not played or attended any practices. Absolutely no refunds will be given if the child has received a uniform. Special consideration may be given to children who are medically unable to play. Requests will be looked at on a case by case basis and must be approved by the board. Refunds will only be considered with a written request. The request must include the child's name, date of birth and a brief reason for the refund, along with a self addressed stamp envelope. Mail the request to the following address:
